This is to certify that insurance has been waived by you, the undersigned, and should any damage (i.e. loss or theft or damage to the goods) be incurred during the time in which Beverly Packing is transporting or storing goods, or providing services for you under our agreement, you agree to release Beverly Packing, INC from liability for damage, loss, or any other claim that may arise pertaining to the goods in question. |

In the absence of the foregoing waiver, please note that the U.S. Department of Transportation, in the case of loss or damage, requires maximum liability of $0.60 per pound, per article, which is provided at no additional cost to the client. |
